Johnny ha estado creando oportunidades de negocios para The Lost en Liberty City, pero su primera lealtad debe ser el parche que lleva en la espalda y con Billy Grey, el presidente del club. Sin embargo, cuando Billy regresa de rehabilitación empeñado en derramamiento de sangre y desenfreno, Johnny se encuentra en medio de una guerra feroz con bandas rivales por el control de una ciudad desgarrada por la violencia y la corrupción. ¿Puede sobrevivir la hermandad?.
